The Acrobat Reader allows you to download
files from this site that have been written in Portable Document Format.
PDF files are faster to download than most other file types, and because
PDF forms keep fonts and images intact, they will look exactly the same as
the forms that are mailed to the Chapters.
By installing the Acrobat Reader you will be able to download and view
forms and print them no matter what browser or operating system you are
